InFANT Funeral & Burial Assistance outreach

Because of the sudden and unexpected nature of infant loss, many families find themselves facing a great deal of financial hardship as they prepare to lay their little one to rest. Through completion of this form, a request can be made to the Missing Alexis Foundation to help offset some of these costs directly with the funeral home or hospital.

Consideration is given to every application, however, scholarships for funeral costs are alloted based on donation availability. While we wish to assist every family in need, please understand that circumstances may not always make this possible.
